Please review our Handbook and the North Carolina Child Care Law for Family Child Care Homes prior to applying to our program.  To complete the application, you will need to consent that you have reviewed both of these documents, which are attached to this email.

 

I have also attached Medical Report and Immunization History forms that will need to be completed by you and your child's physician.  Please be sure to return this prior to the first day of enrollment.
